Beforethe grading scheme varied between examination boards, but typically there were "pass" grades of 1 to 6 and "fail" grades of 7 to 9. However the grades were not displayed on certificates. The CSE was graded on a numerical scale from 1 to 5, with 1 being the highest, and 5 being the lowest passing grade. Below 5 there was a U ungraded grade.

I'd highly recommend getting the syllabus and just going through each point on that, making sure you cover it all in enough detail. If you don't know where to find it, ask your teacher, they should have some sort of guide to help you through it.
Make sure you use relevant source material and comment on whether the sources are reliable or not. For example, a study from a doctor will be reliable because they are within the profession, however a wikipedia source may not be as reliable as it can be widely edited.
That's what I did when commenting on quotes etc.
As for structure, I'm not sure how your teacher wants you to do it, but what we basically did was write about each of the points that need to be covered.
Giving opinions, etc, showing sources. Then comparing and contrasting the points, with opinions. Then finally writing about how reliable we thought the sources were.
